Your First Visit

Welcome to our office and thank you for trusting us with your care.
We are open for both in-person and phone appointments by referral only. Once a referral is received by our office, we triage it based on urgency and will call you directly to book your appointment.

Forms

Whether you are coming in person or joining us for a telehealth appointment, please complete the patient history form before your visit.


You can either print the form and bring to your appointment, or fill online and email the form to your doctor.

There will also be printed copies available at the office. Please arrive with some extra time to allow you to complete it before your booked appointment. 

Referrals

In order for your care to be covered by Medical Services Plan, you need to be referred to our specialists by your family doctor or midwife.

 

Please ask your family doctor to clearly indicate the nature of your referral to help us triage it appropriately. We save blocks in our doctors’ schedules for urgent appointments, and can see you quickly if your case is urgent.

 

Please make sure your contact information is up-to-date with your family doctor so that we can contact you easily.

Your Appointment

Please arrive at least 15 minutes prior to your scheduled appointment time to allow time to check-in with our office staff and complete any outstanding forms. Parking is available underground at the Altus Building. Access is from the south side of the building. Please proceed down the ramp, keeping right, to the yellow marked section for commercial visitors and take the elevator to floor 2.

 

Please also allow for your appointment running longer than expected. As specialists, we may be called to the hospital for emergencies. This is not within our control. If this does happen, we’ll do our best to accommodate you as soon as possible — often the same day as your appointment or the next day.

Remember to bring your B.C. Services Card to your appointment. You may have combined this with your driver’s licence, or you may have a separate card. It has your Personal Health Number on the back. CareCards have not been issued since 2013, and are no longer accepted for provincially-funded services without additional photo ID. If you don't have MSP coverage, private fee rates will apply to appointments provided, and will be invoiced to you directly.

It is helpful if you advise our office staff if you have had any lab tests, ultrasounds, or any changes in your health history since your last appointment.

Cancellation Policy

Missed appointments delay care for everyone. We require at least two business days' notice for changing or cancelling appointments, so that we can allocate your appointment slot to another patient. Fees apply to late cancellations and missed appointments.
